Welcome. 
This is a tactical briefing, not a standard coaching intake.
To ensure our 90-minute session is used strictly for strategy and not fact-finding, please complete this form thoroughly. Be specific, factual, and direct. Your answers will allow me to draft the initial sections of your Crisis Protocol before we meet, so that we can finalize it by the end of our session together. 

Confidentiality Note: All information is confidential. However, if you disclose immediate danger to yourself, your child, or others, or suspected abuse, I am ethically and legally obligated to take protective action.



Intake Application Questions

Once your submission is received:  I will review your answers immediately and email my response to you (generally within 12 hours), regarding your application for my High Stakes Strategy Session. 


Section 1: THE CORE CRISIS
Q1: In one sentence, what is the primary crisis you are facing right now? ( e.g., "Agency threatening removal," "School suspension pending expulsion.")
Q2: What is the specific deadline associated with this crisis?
Q3: What is the worst-case scenario you are afraid will happen if we do nothing?
Section 2: TIMELINE & KEY PLAYERS
Q4: List the last 3-5 critical events in chronological order. Include dates, who was involved, and the outcome. ( e.g., "Oct 12: Caseworker called... Oct 14: Sent email...")
Q5: Who are the key players currently involved? (Names/Roles)
Q6: Have you already taken any legal or formal action?
Section 3: COMMUNICATION AUDIT (Critical)
Q7: What is the most recent communication you had with the opposing party? (Paste email/text or attach file)*

Q9: Have you been advised NOT to speak to anyone?

Q11: What is the current status of the other parent/spouse? (Aligned? Part of the problem? Overwhelmed?)
Q12: What is the ONE thing that must stay stable in the home this week regardless of the crisis?
